Most conversations about AI in business still picture a human reviewing an output before anything consequential happens. A draft gets approved. A recommendation gets accepted. A decision gets confirmed. That model is quietly becoming obsolete. A new generation of agentic AI systems — tools that autonomously chain together multiple tasks, call external services, and act on their own intermediate outputs — are beginning to appear in enterprise environments. They do not wait for permission at each step. That is precisely the point of them, and precisely why they introduce a category of risk that most UK organisations are not yet structured to manage.

The urgency here is not theoretical. As these systems move from developer sandboxes into production workflows — handling supplier communications, triaging customer queries, initiating transactions — the question of where human oversight actually sits becomes a compliance and governance question, not merely an architectural one. For organisations operating under FCA rules or handling personal data subject to ICO accountability requirements, the stakes of getting this wrong are significant and the window to get ahead of it is narrowing.

What Agentic AI Actually Does Differently

Traditional automation — RPA, rule-based workflows, even conventional machine learning pipelines — operates within tightly defined boundaries. Each step is predictable, auditable, and typically designed with a human checkpoint at moments of consequence. Agentic AI breaks that pattern deliberately. Systems built on frameworks such as AutoGPT, LangGraph, or Microsoft's AutoGen are designed to decompose a high-level goal into sub-tasks, execute those sub-tasks using tools and APIs, evaluate the results, and continue — all without pausing for human review between steps. The human sets the objective; the agent determines and executes the path.

In practice, this means an agentic system asked to 'resolve this customer complaint and update the account accordingly' might search transaction history, draft and send a response, apply a goodwill credit, and log the case closure — as a single uninterrupted sequence. Each individual action might appear routine. Taken together, they constitute a decision that, in a regulated context, carries real accountability. The efficiency gains are genuine. But so is the accountability gap that opens when no human reviewed the chain before it completed.

Where FCA and ICO Accountability Rules Create Hard Boundaries

UK regulatory frameworks were not written with agentic AI in mind, but they apply to it regardless. Under the FCA's Consumer Duty, firms must be able to demonstrate that outcomes for retail customers are actively monitored and that decisions affecting those customers are fair, explainable, and attributable to a responsible person or process. An agentic system that autonomously determines a customer's eligibility for a product, communicates that decision, and records the outcome has, in regulatory terms, made a regulated decision. The absence of a human in that loop does not reduce the firm's liability — it concentrates it.

The ICO's accountability principle under UK GDPR presents a parallel challenge. Organisations must be able to demonstrate compliance, which requires knowing what decisions were made, on what basis, and with what data. Agentic systems that call multiple APIs, retrieve and process personal data across steps, and generate outputs that feed subsequent actions can make that audit trail genuinely difficult to reconstruct after the fact. There is also the specific obligation under Article 22 — the right not to be subject to solely automated decision-making that produces significant effects — which agentic pipelines may trigger in ways that a single-step classifier would not, precisely because the cumulative effect of chained decisions can be significant even when no individual step appears consequential.

Redesigning Approval Architecture for Agentic Workflows

The temptation when deploying agentic AI is to treat human oversight as a drag on efficiency — the thing you are trying to automate away. The more productive frame is to treat it as an engineering constraint that must be explicitly designed around. That means mapping your workflows not by what is convenient to automate, but by where accountability is legally or operationally required to be human. Financial approvals above defined thresholds, customer communications that constitute regulated decisions, any action that modifies a record in a way that triggers regulatory reporting — these are natural insertion points for mandatory human gates, regardless of what the agent could do autonomously.

Practically, this requires working through three questions for every proposed agentic workflow. First: what is the worst credible outcome if this chain executes incorrectly, and who is accountable for that outcome? Second: at which point in the chain does the decision become irreversible or externally visible — and is there a human checkpoint before that point? Third: what does the audit trail look like, and can it satisfy a regulator or a subject access request? Teams that work through these questions before deployment, rather than retrofitting governance after an incident, are consistently in a stronger position.

Governance Frameworks Are Lagging — But Not Absent

It would be a mistake to conclude that the regulatory environment offers no guidance simply because it predates agentic AI. The FCA's existing expectations around model risk management, algorithmic accountability, and senior manager responsibility under SMCR provide a workable foundation — one that places clear personal accountability on named individuals for the performance of automated systems. The ICO's guidance on AI and data protection, while written with simpler systems in mind, establishes principles around transparency and data minimisation that apply directly to how agentic pipelines are architected.

What is genuinely absent is sector-specific guidance on agentic systems specifically, and that gap is unlikely to be filled quickly. The practical implication is that organisations deploying these systems now are, in effect, setting their own standards — and those standards will be judged retrospectively if something goes wrong. That is not a reason to avoid agentic AI; the productivity and capability advantages are real and competitors will move. It is a reason to document your governance decisions carefully, involve your legal and compliance teams early, and treat your approval architecture as a first-class design decision rather than an afterthought.

The organisations that will deploy agentic AI most successfully over the next two to three years are not those that automate most aggressively — they are those that are clearest about where human accountability is genuinely required and build that clarity into their systems from the outset. That means revisiting your automated workflows now, before agentic capabilities are layered on top of them, and asking honestly whether your current approval architecture was designed for a world where AI can act autonomously across a chain of consequential steps.
